2. Purpose of the Site
The Site is operated by Perry Family Dentistry, 2017 Continental Pl #9, Mount Vernon, Washington 98273 (“Practice”). It collects contact and screening details (name, phone, email, self-reported credit score, dental history) to determine whether you may be scheduled for a dental-implant consultation at our office. 3. No Doctor-Patient Relationship
Submitting your information, speaking with our staff, or receiving a consultation invitation does not create a doctor-patient relationship. A relationship is formed only after you have completed an in-person clinical examination and signed our standard treatment consent documents.
4. Accuracy of Information
You certify that all information you provide is true and complete. Misrepresentation may result in denial of services.
5. Self-Reported Credit Score
We rely solely on the credit score you enter. The Practice performs no hard or soft credit inquiries. You understand that financing offers discussed at or after your consultation may require a separate credit check by third-party lenders subject to their own terms.
6. Consent to Contact (TCPA & CAN-SPAM)
By checking the consent box and clicking Submit, you:

7. Privacy and HIPAA Notice
Your submissions may contain Protected Health Information (“PHI”). We apply administrative, technical, and physical safeguards that meet or exceed HIPAA standards. PHI is stored in an encrypted, access-controlled practice management system and is used only to evaluate candidacy, schedule consultations, or provide care. 8. Use of Third-Party Platforms
The Site is hosted on a HIPAA-capable marketing platform (currently Go High Level). That platform may collect metadata such as IP addresses and device identifiers. It processes your data only under our written Business Associate Agreement and may not use it for any other purpose.
9. Disclaimers
No Guarantee of Treatment or Outcome. Completing the form does not guarantee you will receive treatment, financing, or any specific result.
Emergency Care. The Site is not designed for urgent or emergent dental issues. If you have pain, trauma, or infection, call 911 or visit an emergency dental clinic.
10. Limitation of Liability
To the maximum extent permitted by Washington law, the Practice shall not be liable for any indirect, incidental, special, or consequential damages arising out of or related to your use of the Site or any delay in scheduling.
11. Indemnification
You agree to indemnify, defend, and hold harmless Perry Family Dentistry, its owners, employees, and agents from any claims or damages resulting from your violation of these Terms or your misuse of the Site.
12. Dispute Resolution
Any dispute arising under these Terms shall be resolved by binding arbitration in Skagit County, Washington, under the Commercial Arbitration Rules of the American Arbitration Association. You waive any right to participate in a class or representative action.
13. Governing Law
These Terms and any dispute hereunder are governed by the laws of the State of Washington without regard to conflict-of-law rules.
14. Changes to Terms
We may amend these Terms at any time. Material changes will be posted on this page with a new Effective Date. Continued use of the Site after changes constitutes acceptance.
